The azalides, part of the macrolide family of antimicrobials (they include azithromycin), have immune-modulating and anti-inflammatory properties, so perhaps that is why they have favorable effects on infection and the inflammatory cascade with infection by SARS-CoV-2. There may be other mechanisms at work.
